Application for admission to Residential Treatment or the on-campus Group Home is initiated by Ozanam's receipt of the following referral material:

• Completed application form
• Social history
• Psychological testing results (WISC-IV and projectives)
• Educational reports (transcripts, behavior and attendance records, immunizations, and IEP information if applicable)
• Physical examination results and immunization report
• Current medication (psychotropic and other)
• Reports of previous evaluation and/or treatment programs

The above information is screened by the Director of Admissions and other members of the Admissions Committee to determine if the youth is appropriate for placement at Ozanam. If the youth is accepted for treatment, a number of formal and informal evaluations are completed to assist in the formulation of an individualized treatment plan that will outline the goals the youth will pursue while at Ozanam, as well as discharge criteria.

If the youth does not appear appropriate based on the referral information, the Director of Admissions or other clinical staff will make recommendations for services that may better meet the needs of the youth.
